Director, Care Integration and Community Outreach

Responsibilities

To lead in the development and implementation of strategies and programmes to facilitate the integration and continuity of care within the organization and the healthcare system, as well as to promote community outreach
Development and implementation of strategies and programmes
- Lead in the development of strategies and programmes necessary to facilitate
  - Care integration and continuity of care for our clients
  - Caregiver support
  - Community outreach
Lead in the implementation and operationalisation of the necessary programmes
Network with external organisations relevant to care integration and community outreach, including being an advocate for care integration and community outreach
Management and leadership of departments
Head the departments relevant to the scope of work above as assigned by the RO, performing tasks which include:
- Developing strategies and workplans for the department
- Managing the staff, including their training and welfare
- Managing the operations of the department

Requirements

Bachelor’s Degree holder
At least 5 years in the Intermediate and Long Term Care sector and in a relevant area of work e.g., social work, other allied health, medical, nursing
Excellent interpersonal skills to work with people at all levels
Excellent written and spoken communication skills
Highly organized and self-motivated
Excellent project management skills
Proficient in MS Office applications
